This internal report analysis Crew 1 by SpaceX on 16 Nov 2020 for NASA marking it return of human space flight in America. In reality, a fake and fabricated video was shown. This report, which is part of lecture notes of on going Master in Engineering in Space Engineering program at MIT, talk about it in detail. It also discusses other recent so called unsuccessful outer and deep space missions (e.g. NASA Mars Rover 2020 launched by ULA on pencil rocket) and elaborately discusses blunders in their design, manufacturing, inspection and testing and operation.
